Fatal Ohio State Fair ride malfunction caused by 'excessive corrosion'

The company that provided the ride to the Ohio State Fair, Amusements of America, described the Fire Ball as an "aggressive thrill" ride. KMG asked all owners of similar rides to shut them down pending an investigation.

In a statement posted Sunday on Facebook, KMG, the ride's Dutch manufacturer, said the failed ride was 18 years old.

KMG said it sent a team of investigators to inspect the ride in Columbus, Ohio, and reviewed security footage of the fatal incident. "On the Afterburner in OH we did the maintenance until a few years ago", Kroon said at the time. Franklin County Coroner Anahi Ortiz said that Jarrell died of blunt force trauma injuries to the head, trunk and lower extremities. He was about to begin his senior year in high school and had enlisted in the U.S. Marine Corps the week before he was killed. A similar ride at the Orange County Fair in California was also shut down to undergo a re-inspection after the OH incident, ABC station KABC in Los Angeles said.

Despite the manufacturer's claims, previous reports from the Cleveland.com note that the ride had been inspected at least three times both by state officials and a third party, and was cleared for operation when the fair was set up prior to opening. Local Cincinnati news station WCPO reported that Jarell's family plans to file a wrongful death lawsuit. Sunday was the fair's last day.
